Age: 55 years

mri

Tumor slightly high on T2 and FLAIR, with vivid contrast enhancement, partially obliterating the right middle ear, extending through the hypotympanum, surrounding the cochlear promontory until reaching the carotid canal. It contours and spares the cochlea. The mastoid antrum is obliterated causing accumulation of fluid on the right.

Case Discussion

The imaging features are those of a tympanic paraganglioma (formerly known as glomus tympanicum), which is the most common tumor of the middle ear cavity. Unfortunately, this case was not confirmed histologically and this diagnosis is just supported by the radiological characteristics.
