Presentation

Patient Data

Age: 25 years
Gender: Female
ultrasound

Diffuse circumferential enlargement of the right ovary with a heterogeneous tumoral involvement displaying multifocal hypoechoic to anechoic cystic areas in the background of echoic solid parenchymal areas. The lesion measures up to an approximate 97 ml in volume, is not hypervascular on color Doppler interrogation. It enhances posteriorly and is thick-walled.

The contralateral left ovary and the anteverted and non-gravid uterus grossly looks normal. No pelvic adenopathy. The liver surface grossly is smooth save for a target-like, tiny solitary hyperechogenic focus (segment VIII medially) that peripherally exhibits mild hypoechoicity.

Case Discussion

The patient has known right sided granulosa cell tumor of the ovary, presenting sonographically as a heterogeneous mass.

Mild loculated and septated clear anechoic fluid (early ascites) is noted as well ipsilaterally within the right lower abdominal quadrant.
