Greenstick fracture of the 5th metacarpal

Presentation

Pain and swelling to the left hand.

Patient Data

Age: 13 years
Gender: Male

Acute incomplete fracture through the mid-diaphysis of the left fifth metacarpal. The cortex on the medial side of the fracture remains partially intact, compatible with a greenstick fracture (yellow arrow).

Suspected acute avulsion fracture involving the base of the fifth metacarpal along the ulnar side, where there is a small bony fragment (blue arrow).

Case Discussion

Greenstick fractures are more common in pediatrics due to the features of immature bone. The greater amount of cartilage and increased bony compliance in children allow for more bowing and bending compared to adults 2.

This case involves a metacarpal with a clear fracture of the lateral side and bowing of the medial side, with an intact medial cortex and periosteum.
