Presentation

Gunshot wound.

Paperclip indicated entry wound and overlies and right lower chest. 

Bullet terminates in the right posterior thoracic soft tissues. 

Path of bullet: Right middle lobe injury, through the liver with large laceration, body wall. 

Right chest tube with medium hemothorax. 

Case Discussion

This case illustrates how understanding the pathway of the bullet assists in quickly identifying and understanding the pattern of injury. 

The scout images can be particularly helpful when looking for paperclips indicating the site of entry/exit wounds.
