Presentation

Patient has abdominal pain, proteinuria, anorexia and vomiting

Age: 15 years
Gender: Female

A right ovarian homogeneous hyperechoic lesion, suggestive of a hemorrhagic ovarian cyst. Minimal free fluid at the pelvis noted.

This patient went on to have surgical resection, that revealed hemorrhagic corpus luteal cyst on histopathology.

Case Discussion

A hemorrhagic ovarian cyst usually results from hemorrhage into a corpus luteum or other functional cyst.

Differential considerations on ultrasound include:

  • cystic ovarian neoplasm: the most helpful feature in distinguishing hemorrhagic cysts from ovarian neoplasms is the presence of papillary projections and nodular septa in the latter.
  • endometrioma: typically contains uniform low-level internal echoes
