HSV encephalitis

Presentation

Unwitnessed collapse with seizures. Febrile.

Patient Data

Age: 60 years
Gender: Male

Subtle hypoattenuation and loss of grey-white differentiation in the medial portion of the right anterior temporal lobe. The diagnosis not to be missed in this setting is that of HSV encephalitis and urgent MRI and CSF sampling should be sought. 

MRI done two days after

T2 and DWI hyperintensity within the right medial temporal lobe including the hippocampus. 

7 months later

Right temporal lobe encephalomalacia at site of previous HSV encephalitis.
