Hyperflexion-compression fracture of the 1st thoracic vertebra

The patient hit her head on a cruise ship in rough seas 3 weeks ago and complained of aggravating neck pain, numbness and weakness in her right hand.

Patient Data

Age: 60
Gender: Female

Unstable burst fracture with osseous material protruding into the spinal canal and the right neuroforamen.

Case Discussion

If you look carefully at the T1 and STIR images, you must suspect an injury of the right interarticular portion and inferior articular process of T1, that would mean involvement of all 3 columns. CT is mandatory for treatment planning in this situation.
