Hypertensive intraparenchymal and intraventricular hemorrhage

Presentation

Hypertensive patient presents with right sided hemiplegia and altered mental status for past 24 hours.

Patient Data

Age: 65 years
Gender: Male

Large, acute extensive intraparenchymal hematoma in the left cerebral hemisphere deep white matter extending to the ventricular system. There is resultant midline shift of approximately 12 mm toward the right side.

Case Discussion

Hypertensive hemorrhage can occur in the deep white matter, basal ganglia and cerebellar hemispheres. Intraparenchymal hematoma can extend to the ventricular system as in this case.
