Hypertrophic pyloric stenosis

Diagnosis almost certain

Presentation

Non-bilious projectile vomiting after taking milk. On examination, there is palpated olive-sized mass in the right upper quadrant.

Patient Data

Age: 25 days
Gender: Male
ultrasound

An ultrasound scan through the pylorus of the stomach shows a 25 mm length of the pylorus, pyloric transverse diameter is 13.6 mm and pyloric muscle thickness is 4.3 mm.

A typical antral nipple and cervix sign is seen in image 1. 

Target sign and stomach hyperactivity were also seen (not shown).

Case Discussion

This patient is presented with a typical history of non-bilious projectile vomiting. On examination, there is an olive-like palpable mass in the epigastrium and he was dehydrated. No risk factors were present like tracheo-esophageal fistula, Turner syndrome, trisomy 18, etc.
