Hypoxic ischemic encephalopathy

Diagnosis probable

Presentation

Cardiac arrest 15 minute downtime. Myoclonus. Rule out hypoxic ischemic injury.

Patient Data

Age: 90 years
Gender: Male

There is diffuse obscuration and mild edema of the cerebral and cerebellar hemispheres. There is hypodensity of the bilateral caudate nuclei.

Comparison to a prior exam shows a relatively normal appearance of the caudate nuclei (right arrows).

Case Discussion

Example of hypoxic ischemic encephalopathy/injury. The most striking appearances are diffuse grey white obscuration and hypodense caudate nuclei, which should normally be similar in density to cortical grey matter. The patient was palliated after this exam.
