Presentation

Motorcycle accident. Polytrauma patient.

Age: 14 years
Gender: Male

The iliac crest apophyses cover about half of the iliac crest, which can be classified as a Risser stage 2.

No fractures are noted.

Case Discussion

In the present case, the patient suffered polytrauma with normal pelvic radiographs.

The Risser classification is unrelated to the case itself but assessed for teaching purposes. A Risser stage 2 is normal for a 14-year-old male 2, which is the case.

The Risser classification indicates skeletal maturity, especially that of the vertebrae. Because of this, the classification is widely used in the management of adolescent scoliosis.

The classification is divided into stages 1:

  • stage 0: no ossification center at the level of iliac crest apophysis
  • stage 1: apophysis <25% of the iliac crest
  • stage 2: apophysis between 25-50% of the iliac crest
  • stage 3: apophysis between 50-75% of the iliac crest
  • stage 4: apophysis >75% of the iliac crest
  • stage 5: fused iliac crest apophysis
