Presentation

History of right-sided back/groin pain. Hemophiliac.

Patient Data

Age: 45 years
Gender: Male
mri

A large heterogeneous signal intramuscular non-enhancing hematoma measuring about 70 x 110 x 150 mm along the course of the right-sided iliopsoas muscle is seen. 

Case Discussion

Right iliopsoas non-enhancing hemorrhagic mass lesion consistent with iliopsoas hematoma (hemophilic pseudotumor), in the clinical setting of hemophilia.

The two main differential diagnoses of iliopsoas hematoma (hemophilic pseudotumor) are iliopsoas abscess and iliopsoas tumors such as sarcoma.
