Ruptured intracranial dermoid cyst

Case contributed by Hani Makky Al Salam
Diagnosis probable

Presentation

A young patient with history of headache and seizure of 3 years duration.

A distinctly hypodense mass is seen centered in the left lateral sulcus, with similarly hypodense material seen around it in the CSF spaces in the suprasellar and interpeduncular cisterns, as well as in numerous sulci along the left frontoparietal convexity.

MRI better depicts the irregular internal structure of the fatty lesion, while the ruptured content in the sulci is best seen on the FLAIR sequence.

Case Discussion

Brain CT and MRI demonstrates ruptured dermoid.
