Ischemic stroke - hyperdense MCA sign

Presentation

Dense acute hemiplegia

Patient Data

Age: 90 years
Gender: Female

Hyperdense right distal internal carotid artery and proximal middle cerebral artery in keeping with thrombus.

There is a subtle loss of grey-white matter differentiation involving the right insular cortex as well as the right frontoparietal MCA territory.

The left cerebral hemisphere is unremarkable

No intracranial hemorrhage. Periventricular chronic small vessel ischemia noted. The ventricles and sulci are age-appropriate.
