Presentation

Vomiting and abdominal distension

Patient Data

Age: 60 years
Gender: Female
x-ray

There is centralisation of small bowel loops, with lateral bulging of the properitoneal fat planes. Findings would be in keeping with a moderate-large amount of ascites.

Lobulated soft tissue masses in both adnexal regions. No normal ovaries seen.

Large amount of ascites.

Case Discussion

The main differentials for this case would be between primary ovarian malignancy and ovarian metastases.

Excision of bilateral ovarian masses showed metastatic gastric carcinoma, ie. Krukenberg tumors.

Gastrocopy found poorly differentiated adenocarcinoma within the gastric body (not well appreciated on this CT).
