Presentation

Acute right hemiplegia.

Patient Data

Age: 80 years

Calcified thrombus at the left MCA bifurcation. No other evidence of acute stroke. Old left superior frontal gyrus encephalomalacia. 

Multifocal regions of diffusion restriction in the left MCA territory reflecting acute stroke. 

Case Discussion

Calcified cerebral thrombus is an uncommon cause of ischemic stroke, occuring in ~2.5% of patients. They are often small (2-3 mm) and can be overlooked as old infection, vessel wall calcification or hemorrhage.
