Left-upper lobe partial anomalous pulmonary venous return

Case contributed by Abdulmajid Bawazeer
Diagnosis certain

Presentation

History of quarrel and chest cut wound. (incidental finding).

Patient Data

Age: 40 years
Gender: Male

The left upper pulmonary vein drains through the left vertical vein into left the brachiocephalic vein directly (not into the left atrium).

Case Discussion

CT angiography and 3D CT clearly shows the common upper pulmonary vein connecting to the anomalous vertical vein, which in turn drains into the left innominate vein. The pulmonary veins of the upper lobe may show early and intense opacification due to retrograde flow of the contrast. This will be more obvious when the contrast media is being administered via the left arm. (incidental finding ).
