Presentation

Breathlessness.

Patient Data

Age: 70 years
Gender: Male
x-ray

Coarse reticulonodular opacity throughout the right lung with prominent septal lines to the upper mid zones, appearances suggestive of lymphangitis infiltration. The right hilum is prominent and a nodule is noted adjacent to the hilum. Right mediobasal atelectasis with triangular opacity and loss of diaphragm silhouette. 

Postsurgical changes noted in the left hemithorax, overinflated left lower lobe noted and pleural thickening at the left apex.

Case Discussion

This patient has a known history of metastatic lung adenocarcinoma - this pattern is consistent with lymphangitis carcinomatosis
