Motor vehicle accident. Left leg looks a little shorter than the right.

There are fractures of the superior and inferior left pubic rami, as well as through the left sacrum.

Case Discussion

A Malgaigne fracture comprises of disruption of superior and inferior pubic rami and the ipsilateral sacroiliac joint, or as variants - the ipsilateral sacral wing (as in this case) or ileum.  This results in an essentially disconnected hemipelvis and is regarded as an unstable injury.
