Patient Data
Fracture of the right superior and inferior pubic rami and the right sacral ala. The pubic symphysis remains normally aligned.
Vertical fractures involving the right side of pelvic ring. Above the acetabulum, the oblique fracture in right iliac bone involving its posterior third and start from the iliac crest and exit into the lower one-third of ipsilateral SI joint with involvement of articular surface without its significant diastasis. Below the acetabulum, there are comminuted fractures of ipsilateral superior and inferior pubic rami; however, there is no evidence of fracture of acetabular walls. There is fracture of ipsilateral ischial spine as well. Contralateral hemipelvis is intact without fracture.
Case Discussion
This injury is characterized by rupture of entire pelvic floor, including posterior SI complex as well as sacrospinous and sacrotuberous ligaments.
It can often be recognized clinically as shortening of the ipsilateral lower extremity.
