Unencapsulated normal fat are present diffusely sparse throughout the mediastinum. There is also a fatty liver.

Case Discussion

Mediastinal lipomatosis is a benign entity of increased collections of non encapsulated fat throughout the mediastinum. 

This is an overweight patient with no other known reason for mediastinal lipomatosis.
