Increasing headaches.
A somewhat hyperdense enhancing mass is located peripherally in the right frontal region with extensive vasogenic edema.
MRI demonstrates an extradural mass with heterogenous enhancement is surrounded by extensive vasogenic edema. Other smaller meningiomata are seen at right temporal, right frontal and right parietal para sagittal regions.
Case Discussion
Although brain edema was once thought to represent brain invasion this is now known not to be the case. Compression with venous hypertension, rapid growth and local vasoactive factors are thought to play a part.
At craniotomy, the mass was separate from the brain, with an intact pial layer.
Histology confirmed a meningioma.
