There is a large well-circumscribed broad-based left frontotemporal extra-axial mass (measuring 10 x 7 x 5 cm), isodense to the cortical grey matter on non-contrast images with a relatively homogeneous enhancement on post-contrast images except the central area of cystic changes. Surrounding vasogenic edema with a mass effect on the midline structures and subfalcine and uncal herniations. Adjacent reactive hyperostosis is noted on the bone window.

Bilateral phthisis bulbi with atrophied optic nerves.

Case Discussion

MRI features most consistent with a frontotemporal meningioma with double cerebral herniation ( subfalcine and uncal herniation).
