Mesial temporal sclerosis: right

Marked asymmetry of the hippocampal heads with an atrophic right side. Additionally the right hippocampus demonstrates high signal on FLAIR. Features are consistent with mesial temporal sclerosis

Case Discussion

The patient went onto biopsy. 

Final Diagnosis: Brain, temporal: Right mesial temporal sclerosis.

Microscopic Description: Sections of the mesial structures show extensive mesial temporal sclerosis, represented by neuronal loss in sector CA1, with less mild neuronal loss in sectors CA3 and CA4
