Diagnosis probable

Presentation

Left pleuritic chest pain. weight loss. Past history asbestos exposure.

Patient Data

Age: 70
Gender: Male
ct

Large left pleural effusion. Thick pleural rind encasing total left lung and involving the left major fissure Calcified pleural plaques are present.

No mediastinal nodes. No lung mass . There are no changes of lung fibrosis.

Conclusion:

The appearance in a patient with prior asbestos exposure are most likely due to mesothelioma,

Case Discussion

In a patient with asbestosis exposure and these CT findings mesothelioma is by far the most likely diagnosis.The presence of calcified pleural plaques strengthens this diagnosis.

In a person with no prior exposure to asbestosis similar CT findings can be seen in pleural metastases.

Pleural TB can result in marked pleural thickening encasing the lung.
