Metaplastic carcinoma - breast

Notice how the lesion is high density and has areas of spiculation and well-defined margins in places, in the same lesion. These two findings, if present, are highly suggestive of metaplastic carcinoma.
On the annotated image, the arrows show "smooth" margins between the areas of spiculated margins. Quite strange once you start to pick up on it.

The mixed echogenicity is a result of focal areas of necrosis and hemorrhage.
