Metastasizing pleomorphic adenoma

Diagnosis almost certain

Presentation

Had surgery for left parotid mass 4 years prior than for recurrence 2 years later.

Patient Data

Age: 25 years
Gender: Female

Enlarged liver with multiple heterogeneously enhancing masses of various sizes, the largest mass is located in the right lobe measuring 17 x 16 x 13 cm.

Bilateral heterogeneously enhancing renal masses of various sizes, the largest on the left measuring 14.4 x 12.3 x 9.2 cm, displacing stomach and corporeo-caudal segment of the pancreas anteriorly and spleen laterally.

Small celiac and para-aortic lymphadenopathy.

Minimal ascites with mild omental fat stranding and thickening of the peritoneal reflections.

No adrenal lesion seen

No mediastinal or hilar lymphadenopathy.

Both lungs are clear.

No bone lesion was seen on the bone window (not shown).

Case Discussion

CT features of multiple hepatic and renal masses in a patient operated twice, for parotid pleomorphic adenoma 4 years ago than for local recurrence 2 years later.

The patient went to have a hepatic biopsy with a histopathological exam that confirm the diagnosis of metastatic pleomorphic adenoma.

Pleomorphic adenoma is the most common salivary gland tumor with 84% in the parotid gland.

 Metastasizing pleomorphic adenoma is the third type of malignant mixed tumor of salivary glands and is the rarest. It presents with metastases to lung, bone, and soft tissues despite having 'benign' histology. Metastases in the multiple organs such as liver, kidneys, lungs were also described in the literature 1.
