Metastatic esophageal squamous cell carcinoma

Diagnosis almost certain

Presentation

Chest pain and dysphagia.

Patient Data

Age: 70 years
Gender: Female

Marked circumferential increased wall thickness due to tumoral infiltration is present at the distal half of the esophagus, measuring about 12 cm in length. The mass is in close contact with the adjacent pericardium, but the arc of the contact between the mass and aorta is less than 90 degrees.

Several enlarged lymph nodes with short axis diameter up to 27 mm are seen in the vicinity of the diseased segment. In addition, several enlarged lymph nodes are seen in upper perigastric and paracardia regions with short axis diameter up to 30 mm.

Several calcified nodules up to 22 mm and a few scattered atelectatic/fibrotic bands are observed in both lungs, particularly in the upper lobes.

Three ill defined low enhancing masses are seen at the liver up to 42 mm, inferring metastases.

Case Discussion

Esophageal mass; pathology proved squamous cell carcinoma with mediastinal and upper perigastric enlarged lymph nodes and liver metastases.
