Migrated intrauterine contraceptive device

Case contributed by Karwan T. Khoshnaw
Diagnosis almost certain

Presentation

Missing IUCD string.

Patient Data

Age: 30 years
Gender: Female

Normal sized uterus showing a migration of an IUCD outside the uterus to the peritoneal cavity (just posterior to the urinary bladder and anterior to the uterus, in the anterior cul-de-sac) with a normal empty endometrial cavity/ The findings are consistent with an IUCD related silent uterine perforation.

Case Discussion

The incidence of transuterine perforation and migration of intrauterine contraceptive devices (IUCDs) into the abdominal cavity has been estimated at less than 0.1%. Complete uterine perforation, in which the IUD is partially or completely within the peritoneal cavity, requires surgical management.
