Monteggia fracture-dislocation

There is a comminuted fracture of the mid-diaphysis of the ulna, with radial offset and ulnar angulation of the distal ulna fragment (fracture apex lateral). The radial head is dislocated anteriorly. 

Overlying casting material is noted.
