Multiple cerebral infarcts - cardiogenic emboli

Multiple small cortical/subcortical lesions of high signal on FLAIR and T2 with restricted diffusion, scattered throughout both cerebellar and cerebral hemispheres with no hemorrhagic component seen on T2* in keeping with multiple infarcts.
Generalized cerebellar and cerebral volume loss with chronic small vessel ischemic changes.
Case Discussion
The clinical presentation and the MRI features are most consistent with multiple acute cerebellar and cerebral infarcts of cardiac origin (cardiogenic emboli).
