Neonatal pneumothorax

Case contributed by Jeremy Jones
Diagnosis almost certain

Presentation

Respiratory distress.

Patient Data

Age: 3 days
Gender: Male
x-ray

Relatively lordotic film with low-volume lungs with diffuse granular opacities and a superimposed right sided pneumothorax.

Case Discussion

The lung volume is low despite an appropriate number of posterior ribs because of the relatively lordotic view. There is no ossification of the humeral heads suggesting prematurity.

The diffuse granular pulmonary opacification is consistent with RDS and there is a superimposed right sided pneumothorax and a small basal pneumothorax on the left also.
