Multiple patchy region of increased T2 signal are demonstrated associated with contrast enhancement. Some of the these are periventricular, some are subcortical and others are in the brainstem.

4 years later enhancement persists but has altered in appearance and distribution.

Case Discussion

This patient had systemic sarcoidosis diagnosed on histology from a lung biopsy. Changes are consistent with neurosarcoidosis. The presence of enhancement 4 years later strongly argues against demyelination.
