Presentation
Acute onset of hyperglycemia in the context of undiagnosed diabetes mellitus. Presented with severe chorea movements of all 4 limbs, right worse than left. Slight improvement of chorea after 3 months of symptom onset.

High T1 and low T2/FLAIR signal are seen in the caudate and lentiform nucleus.

Non-ketotic hyperglycemic hemichorea (NHH) is rare, and is one of the causes of T1 bright basal ganglia.
