Normal pressure hydrocephalus

Presentation

Gait disturbance and urinary incontinence.

Patient Data

Age: 76-year-old
Gender: Male

No intracranial hemorrhage or collection. Old right parieto-occipital infarct with ex vacuo dilatation of the trigone of the right lateral ventricle.

Ventricle size is slightly increased and show a narrowed callosal angle.  No evidence of acute ischemia.

No bony abnormality.

Conclusion: No acute injury. Ventricles are prominent and, in this clinical scenario, raises the possibility of normal pressure hydrocephalus. 

Case Discussion

Normal pressure hydrocephalus is a clinical diagnosis achieved when a patient demonstrates complete or partial symptoms of the clinical triad: gait apraxia, urinary incontinence, and dementia. These are corroborated by some radiographic features, including hydrocephalus with some peculiar features such as a narrowed callosal angle, as demonstrated in this case.
