Presentation

Headache.

The annotated images show the normal course of the oculomotor nerve (III) from the midbrain to the cavernous sinus. 

Other findings include an enlarged CSF space around the oculomotor nerve in the lateral wall of the cavernous sinus, enlarged Meckel cave, partially empty sella turcica and prominent subarachnoid space around the optic nerves with vertical tortuosity.

Case Discussion

The enlarged CSF space around the oculomotor nerve makes it easy for identification in the lateral wall of the cavernous sinus.  

The other imaging findings, in this case, are suggestive of idiopathic intracranial hypertension.
