Esophageal perforation post gastroscopy

Diagnosis almost certain

Patient Data

Age: 75 years
Gender: Male

Multiple air-density lucencies can be seen along the mediastinum and along the left heart border indicate pneumomediastinum.

Multiple extraluminal gas locules can be identified in mediastinum particular anterior to pericardium, around esophagus and ascending aorta. There is perforation of oesophgeal wall can be seen around level of T9 vertebrae. 

Mild pleural effusion can be seen in right lung.

Case Discussion

Esophageal perforation is uncommon post endoscopy but is a known complication. There is a high mortality rate (~20%) so recognition is important.
