Presentation

Crush injury.

Patient Data

Age: 60 years
Gender: Male
x-ray

Pelvic binder and overlying dressings are noted. Pubic symphysis diastasis. Right acetabular, and right superior and inferior pubic rami fractures. Sacroiliac joints are not overtly widened.

Pubic symphysis diastasis. Comminuted fracture of the right ischial tuberosity and displaced fractures of the right inferior and superior pubic rami, extending to the acetabulum. Sacroiliac joints are congruent. Right L4 transverse process fracture. 

Case Discussion

Open book pelvic fractures are a serious injury. They can be life-threatening in the context of uncontrollable pelvic bleeding, which may require treatment with emergent pelvic fixation or angio-embolization. These fractures can also cause urethral injuries.
