Presentation

Motorcycle accident. Unstable pelvis upon physical examination.

Patient Data

Age: 40 years
Gender: Male
x-ray

Pelvic radiograph revealed an open book pelvic fracture as shown by the marked pubic symphysis diastasis (32 mm).

There is no significant diastasis of the sacroiliac joints.

Case Discussion

Open book pelvic fractures are usually caused by high-energy trauma and are commonly associated with life-threatening vascular injuries.

The great degree of pubic symphysis diastasis may be distracting, but it remains important to review the whole pelvis, especially the sacroiliac joints, which are oftentimes injured as well.

In the present case, although no important diastases of the sacroiliac joints were observed, when the pubic symphysis diastasis is >2,5 cm, the anterior sacroiliac ligaments are usually disrupted.
