Open book pelvic injury

Case contributed by Andrew Dixon
Diagnosis certain

Presentation

High speed car accident.

Patient Data

Age: 35
Gender: Male

Trauma Series

x-ray

Open book pelvic injury with diastasis of the pubic symphysis and widening of the right sacro-iliac joint. The pubic diastasis partially reduces following application of a pelvic binder despite it being somewhat superiorly located compared to the ideal position around the greater trochanters.

Post op

x-ray

Internal plate and screw fixation of the pubic symphysis and screw through the right sacro-iliac joint. Anatomic alignment is restored.

Case Discussion

This case demonstrates the appearance of an open book pelvic injury and how application of a pelvic binder can partly restore alignment which can help minimize pelvic bleeding although the evidence base for this is currently limited.
