Presentation
Facial trauma and left orbital pain, limitation of movement of the left eye, and diplopia.
Patient Data
Displaced comminuted blow-out fracture in the left orbital medial wall with partially entrapped superior oblique muscle and medial rectus muscle within fracture gap and related orbital fat hemorrhage are seen. Another blow-out fracture in the nasal side of the left orbital floor with inferior rectus muscle entrapment is also seen. A small fracture line in the left orbit roof from the middle to the nasal side is noted. Hemorrhages within ethmoid and frontal sinuses are seen. Fluid and thick mucosa in other paranasal sinuses more in the right maxillary sinus antrum are noted. Preseptal soft tissue swelling is seen.
Case Discussion
This case is a rare concomitant entrapment of superior oblique muscle and medial rectus muscle within orbital medial wall blow-out fracture gap and inferior rectus muscle within orbital floor blow-out fracture gap.
