Orbital Blow-out fracture and retrobulbar hemorrhage

Presentation

Left side facial trauma and left eye pain and limitation of motion of the eye.

Patient Data

Age: 15 years
Gender: Male

Left orbital medial wall and floor blow-out fracture with pronounced inferior rectus muscle and orbital fat hernia within the fracture gap of the floor, herniated medial rectus muscle within the medial wall fracture gap, and foci of intraconal, retrobulbar, and perineural hematoma. Left side preseptal and lacrimal gland soft tissue swelling and right eye exodeviation and hemorrhage within let side maxillary and ethmoid sinuses are noted.

Case Discussion

This is a rare case of the concomitant orbital medial wall and floor blow-out fracture, herniation of medial rectus muscle, inferior rectus muscle, and orbital fat within fracture gap and associated retrobulbar, intraconal, and perineural hemorrhage.
