Presentation

Facial trauma and the left eye upward gaze limitation.

Patient Data

Age: 45 years
Gender: Female

Left orbital floor blow-out fracture with orbital fat and complete inferior rectus muscle herniation within the fracture gap and hemorrhage within maxillary sinus antrum are seen. Preseptal and lacrimal gland soft tissue swelling and left eye exodeviation and mild retrobulbar hemorrhage are noted. Bilateral proptosis more on the right side is noted.

Case Discussion

The case illustrates the typical non-contrast MDCT features of orbital floor blow-out fracture with inferior rectus muscle entrapment within fracture gap defect. Entrapment of the extraocular muscles can lead to muscle necrosis and is one of the surgical treatment indications.
