Orbital medial wall blowout fracture and medial rectus muscle transection

Presentation

Left side facial trauma and left eye limitation of motion and upper eyelid swelling.

Patient Data

Age: 55 years
Gender: Female

A blowout fracture with a large fracture gap in the left orbit medial wall is seen. The medial rectus muscle and orbital fat have been significantly entrapped within the gap and transection of the posterior segment of the muscle is seen. Remarkable emphysema in the upper eyelid and postseptal soft tissues more on the nasal side is noted.

Case Discussion

The case illustrates the orbital medial wall blow-out fracture with transection of the entrapped medial rectus muscle which is an uncommon complication of this type of fracture.
