On pre-contrast imaging soft tissue density/vascular density tissue fils the orbital apex. On venous phase imaging, this area opacifies like adjacent veins and demonstrates enlargement due to straining (Valsalva maneuver). Note the resulting proptosis. Features are consistent with an orbital venous varix.

Orbital venous varix is an uncommon vascular malformation which is composed of enlarged single or multiple tubular venous channels with direct communication to the systemic venous system.
