Sagittal, axial, and coronal T2WI FS MR images show marrow edema in the anterior aspect of the proximal tibial epiphysis. The marrow edema extends down into the tibial tubercle. There is fluid interposed between the proximal tibia and the distal patellar tendon, denoting deep infrapatellar bursitis. Mild soft tissue swelling is seen anterior to the distal patellar tendon.
Case Discussion
Osgood-Schlatter disease results from repetitive microtrauma to the tibial tubercle during skeletal maturation.
DIFFERENTIAL DIAGNOSIS:
- Normal variant irregular and multiple ossification centers
- no associated soft tissue swelling
- no clinical pain at site
- Patellar tendinosis
- thickened inferior patellar tendon
- MR shows hyperintensity within tendon and adjacent bone on fluid-sensitive sequences
- generally located at the proximal rather than the distal inferior patellar tendon, as opposed to Osgood-Schlatter disease.
