Ostium primum ASD - fetal echocardiography

Presentation

Routine 2nd trimester exam. Nuchal translucency was normal and patient is not in a high-risk group of aneuploidy.

Gender: Female

Ostium primum defect = Ostium primum atrioseptal defect = a subtype of partial atrioseptoventricular defects.

Bidirectional flow through defect.

Normal situs, atrioventricular concordance, well balanced cavities. Output vessels were normal (not shown).

Such a finding leads to screening for associated anomalies and karyotype for Down syndrome.

 

Case Discussion

Ostium Primum ASD is easily diagnosed in fetal echocardiography, while secundum ASD is virtually impossible to diagnose prenatally due to physiologically open vieussens valve through foramen ovale in fetus.

Another subtype of partial ASVD is an inlet VSD.
