Ovarian dermoid cyst - floating ball sign

Well-defined right ovarian complex cystic mass measuring 55 x 48 x 45 mm with two components, the main component is of high signal on T1 and T2, attenuated on fat-saturated sequences (fatty component) and floating ball measuring 35 x 32x 18 mm of intermediate signal on both T1 and T2, partially attenuated on fat-saturated sequences (floating ball sign). Mild peripheral enhancement seen on postcontrast sequences.

MRI features of an ovarian dermoid cyst with intracystic floating ball sign which is uncommon but pathognomonic for ovarian dermoid cyst.
