Paget disease of bone

Case contributed by Jan Frank Gerstenmaier
Diagnosis almost certain

Presentation

Presented with abdominal pain to the ED; bowel obstruction was suspected.

Patient Data

Age: 60 years
Gender: Female

PFA

x-ray

There is no bowel obstruction. There is thickening of the right ileopectineal line, and cortical thickening with increased trabeculation in the right hemipelvis.

Case Discussion

For some unexplained reason, Paget disease of the bony pelvis is more common on the right. Thickening of the ileopectineal line is a specific feature and may be identified when other hallmarks are either not obvious, obscured by bowel or do not extend to the iliac wings. In this case, the whole right hemipelvis is involved.
