Case contributed by Mohammad Taghi Niknejad
Diagnosis almost certain

Presentation

History of pancreatitis and cholecystectomy about 3 months ago.

Patient Data

Age: 55 years
Gender: Male
ct

A 42 mm thin walled nonenhanced cyst is noted at the pancreatic tail. Regarding the patient history, pancreatic pseudocyst is the most likely diagnosis.

The gallbladder is not seen at the anatomical location due to prior resection. Intra and extra hepatic bile ducts are mildly dilated, and CBD measured 9 mm in caliber.

Both kidneys show normal size and position. Several nonenhanced simple cortical cysts are seen at both kidneys, with maximum diameters of 18 mm.

There are several vascular structures in the region of the portal vein inferring cavernous transformation of the portal vein. Some collateral vessels are observed at the upper perigastric and lower para esophageal regions.

Case Discussion

Pancreatic pseudocyst is common sequelae of acute or chronic pancreatitis, and the most common cystic lesion of the pancreas, appears as well-circumscribed, usually round or oval peripancreatic fluid collections homogeneously low attenuation, generally surrounded by a well-defined enhancing wall.
